- We reserve the right to reschedule appointments due to emergencies or provider availability
- Patients arriving more than 15 minutes late may need to reschedule

- Payment is due at the time services are rendered unless prior arrangements are made

- Insurance verification is the patient's responsibility

- Outstanding balances may be sent to collection agencies
- Returned checks will incur additional fees
